Terms

Please be aware that it is your responsibility to know your insurance coverage, deductible, and copay. The full fee will be expected at the time of service.

Anyone who uses insurance coverage for any psychotherapy must be given a diagnosis, as insurance-covered treatment must be deemed “medically necessary.” If you are concerned about a diagnosis remaining on your permanent medical record, or prefer not to frame your concerns in a medical model, I offer the option of paying for services directly. I will discuss the benefits and drawbacks of each choice with you.

Investment in Therapy Services

Quast Counseling Services reserves the right to adjust fees at any time and will notify you in advance if fees are adjusted. These fees may also be adjusted by contract with insurance, managed care organizations, other third party payers, or by agreement with me.

  • Initial Evaluation/Intake (60 minutes) $250.00

  • AODA Assessment (60 minutes) $250.00

  • 53-60 minute Psychotherapy Session $200.00

  • 45 Minute Psychotherapy Session $150.00  

What if you don't accept my insurance?

You have the option to self-pay OR you can check your out-of-network benefits.

Questions to ask your insurance:

  • Do I have out-of-network benefits for my policy?

  • Do I have a mental or behavioral health policy that covers out-of-network benefits?

  • How do I use my out-of-network benefits?

  • Do I have an out-of-network deductible? If so, what is it and how do I meet the deductible?

What’s a superbill? What do I do with it?

A superbill is an invoice I can provide you if I am not in network with your insurance panel. This invoice has information such as dates of sessions, a diagnosis code, and other personal information such as your date of birth and address.

Once you have a superbill and have confirmed your insurance coverage, you can submit this to your insurance for reimbursement. The timeframe and percentage of reimbursement depends solely upon your policy and coverage.

Overall, I want my fees for therapy services to be transparent and affordable. If you have any questions or concerns, please feel free to contact me.

Billing

I require that all clients keep a credit card on file and sign a consent for automatic billing, so that therapy is not disrupted to take care of finances. This card is stored in my secure Electronic Health Record System, Simple Practice. Your card will be charged any copay, deductible, or missed appointment fee within 24 hours following your appointment.

Disclaimer for Good Faith Estimate 

Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” (GFE) of expected charges.

The GFE shows the costs of items and services that are reasonably expected for services provided by Quast Counseling Services. The estimate is based on information known at the time the estimate was created. It does not take into account any reimbursement that you may receive as a result of out of network benefits. 

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. You may contact Courtney Quast to let her know the billed charges are higher than the Good Faith Estimate. You can ask her to update the bill to match the Good Faith Estimate, ask to negotiate the bill, or ask if there is financial assistance available.

Make sure to save a copy or picture of your Good Faith Estimate.
